These terms govern your use of the Zentra Home Care website, phone, text, and enrollment services. By using our site or requesting assistance, you agree to these terms. Please read them carefully.

1. Who we are

Zentra Home Care is a Michigan-based enrollment and care coordination team that helps families qualify for Michigan Medicaid Home Help and related programs. We are not a medical provider, law firm, tax advisor, or agent of the State of Michigan.

2. What our service is — and isn't

  • We do help families understand and apply for Michigan Home Help, the MDHHS-6200 medical certification, and adjacent VA benefits.
  • We do not guarantee approval of any benefit or the dollar amount awarded by the state. Final decisions rest with MDHHS and the VA.
  • We do not provide legal, medical, or tax advice. For those, consult a licensed professional.

3. Your responsibilities

  • Provide accurate and complete information when asked.
  • Notify us of any change in care needs, living situation, or eligibility status.
  • Review any form we fill on your behalf before it is signed and submitted.
  • Do not submit another person's information without their consent.

4. Eligibility and no-guarantee

Eligibility for Michigan Medicaid Home Help and related programs is determined solely by the Michigan Department of Health and Human Services (MDHHS). Our eligibility discussion and guidance is informational. We cannot guarantee that your application will be approved.

5. Fees

Zentra Home Care does not charge families directly for eligibility screening, enrollment assistance, or standard care coordination. Caregiver compensation is paid by Michigan Medicaid to the caregiver based on approved hours. If a service ever requires a fee, we will tell you clearly in writing before any work starts.

6. Electronic signatures

By drawing your signature on our online forms, you agree that the electronic signature has the same legal effect as a handwritten one under the federal E-SIGN Act and Michigan's Uniform Electronic Transactions Act (MCL 450.831 et seq.). You can request a paper-signed copy at any time.

7. Intellectual property

The Zentra Home Care name, logo, text, images, and software on this site are owned by us or our licensors. You may not copy, reproduce, or create derivative works from our site content without written permission. State forms (DHS-390, MDHHS-6200, etc.) are public documents published by MDHHS.

8. Links to other sites

Our site may link to external resources such as MDHHS, the VA, or Michigan Veterans Affairs Agency. We are not responsible for the content or practices of those third-party sites.

9. Communications

By contacting us, you consent to receive replies by phone, text, or email. Standard message and data rates may apply to SMS. Reply STOP to unsubscribe from SMS at any time.

10. Disclaimers

Our website and services are provided on an "as is" and "as available" basis, without warranties of any kind, either express or implied. We do not warrant that the site will be uninterrupted or error-free.

11. Limitation of liability

To the maximum extent permitted by Michigan law, Zentra Home Care is not liable for indirect, incidental, or consequential damages arising from your use of the site or services. Our total liability for any claim is limited to the amount you paid us in the twelve months before the claim (which for most families is zero).

12. Governing law

These terms are governed by the laws of the State of Michigan, without regard to its conflict-of-law rules. Any dispute not resolved informally will be brought in the state or federal courts located in Wayne County, Michigan.

13. Changes to these terms

We may update these terms as our services change. The "Last updated" date at the top reflects the latest version. Continued use of our services after changes means you accept the revised terms.
