1. Information we collect
We collect only what is needed to determine eligibility, complete enrollment, and provide ongoing care coordination. This typically includes:

- Care recipient information — name, date of birth, Medicaid/Recipient ID, living arrangement, and basic care needs described on state forms (DHS-390, MDHHS-6200).
- Caregiver information — name, relationship to the client, and documentation required by Michigan Home Help.
- Communication records — notes from calls, texts, and emails you exchange with our team.
- Electronic signatures — if you sign a form online, we store the drawn signature image, timestamp, and IP address as proof of intent.
- Website usage — basic analytics such as page views, browser type, and referral source.
2. How we use your information
- To determine whether your family qualifies for Michigan Medicaid Home Help or other programs.
- To complete and submit state enrollment forms on your behalf, with your consent.
- To coordinate with MDHHS offices, medical providers, and VA offices when relevant.
- To respond to your questions and provide ongoing support.
- To comply with legal obligations and state Medicaid recordkeeping requirements.
3. Who we share information with
We do not sell your information. We share only what is required to deliver the services you ask for:
- Michigan Department of Health and Human Services (MDHHS) — as part of your Home Help application.
- Medical providers — limited to forms you have signed, such as the MDHHS-6200 medical certification.
- Service providers — secure email, storage, and scheduling tools we use to run our business (all subject to confidentiality obligations).
- Legal authorities — only when required by law.
4. HIPAA and health information
Some of the information you share may include protected health information (PHI). While Zentra Home Care is not a HIPAA-covered entity for all purposes, we follow HIPAA-aligned best practices for any PHI we handle, including encryption at rest and in transit, access controls, and minimum-necessary sharing.
5. How long we keep your information
We retain enrollment records for as long as required by Michigan Medicaid rules (generally six years after the last service date). Non-enrollment inquiries are deleted after 24 months unless you ask us to keep them.
6. Text messages and calls
By providing your phone number, you consent to receive calls and SMS from Zentra Home Care related to your enrollment or care coordination. Message and data rates may apply. Reply STOP at any time to opt out of SMS.
7. Cookies
Our website uses a minimal set of cookies: essential cookies to operate the site, and optional analytics cookies to understand which pages are useful. You can disable cookies in your browser settings; essential site features will still work.
8. Your rights
You can at any time:
- Request a copy of the information we hold about you.
- Ask us to correct anything that is wrong.
- Ask us to delete your information (subject to state Medicaid recordkeeping requirements).
- Withdraw consent for marketing communications.

9. Children
Our services are directed at adults. We do not knowingly collect information from children under 13 except as part of a caregiver enrollment where the care recipient is a minor and the parent or guardian has signed the forms.

We may update this policy as our services change. The "Last updated" date at the top reflects the latest version. Material changes will be communicated by email to enrolled clients.
